Assumption of Risk

I understand that participation in the sewing class involves certain risks, including but not limited to, needle pricks, cuts, burns, and other injuries associated with sewing and the use of sewing equipment. I acknowledge that Bolt Fabric Boutique (to be referred to as Bolt) has taken reasonable precautions to minimize such risks, but I understand that accidents can still occur.

Release of Liability

I hereby release and discharge Bolt, its directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by myself or my child, or to any property belonging to me, whether caused by the negligence of Bolt Fabric Boutique or otherwise, while participating in the sewing class.

Indemnification

I agree to indemnify and hold harmless Bolt, its directors, officers, employees, volunteers, and agents from and against any and all liabilities, claims, demands, actions, or causes of action, including attorney's fees and costs, arising out of or related to my participation in the sewing class.

This waiver and release shall be governed by and construed in accordance with the laws of the state of Oregon.
